Requirements For Availment of Services Programs
Procedures to be Followed

  • Business Permits
  1. Barangay Clearance
  2. Community Tax Certificate
  3. Fire Inspection Certificate
  4. SSS Clearance
  5. Zoning Compliance Certificate (for New Application)
  1. Secure form permit and license division and have it notarized.
  2. Secure tax clearance from the City Treasurers Office, and computation fees and taxes.
  3. Submit application form with all the requirements at Permits and License Division for inspection evaluating and numbering.
  4. Payments of taxes and fees at the city treasurers office.
  5. Present back to the Permits and License Division the Official Reciept for paymets of taxes and fees claim taxpayers copy of business permit for the mayors signature.
  • Issuance of Trisikad and Operators Permits
  1. Community Tax Certificate
  2. Barangay Clearance
  1. Fill up the given form, submit requirements and have it numbered at the permit and license division.
  2. Pay the fees at the city treasurers office.
  3. Return the official reciept to the Permits and License Division and claim ID for driver and plate operator.
  4. Have your permit signed by the executing assistant or City Mayor.

  • Issuance of Working Permit(Food Handlers, Dancers, Masseuse Hostess)
  1. City court clearance
  2. Police Clearance
  3. Mayor's Clearance
  4. Community Tax Certificate
  5. Picture
  1. Submit all requirements and fill up form.
  2. Have it signed by a physician at the City Health Office.
  3. Payment of fees and submit Official Reciept to the Permits and license division.

  • Issuance of Miscellaneous Permits(Dance, Parade, Streamer, Rally, Transport, Educational Trip, Barangay Activities, etc.)
  1. Letter request approved by the City Mayor.
  2. For Parades and Motorcades-need the approval of the City Mayor/City Administrator and Chief of Traffic Police.
  1. Submit all requirements and fill up form.
  2. Have it signed by a physician at the City Health Office.
  3. Payment of fees and submit Official Reciept to the permit license Division.
  4. Payments of taxes and fees at the City Treasurers Office.
