• Filipino Citizen
• 3 pcs. 2x2 ID Picture
• Preferred business name and 3 other alternative business names
• Registration fee of Php300.00
• 3 pcs. Documentary Stamps
• 2 machine copy of voters ID/birth certificate/PRC License if surname is alien sounding.

2. Corporation/Partnership (Optional)
• 2 sets certified true copy of SEC Registration, By-Laws and articles of incorporation.
• 3 pcs. Documentary stamp
• Registration fee Php500.00
• 2 Machine copy of Board Resolution authorizing signatory

II. Mayor’s Permit (License to Operate)
• Business Nmae Registration Certificate
• Brgy. Clearance – 3 copies
• Residence Certificate
• 2x2 ID Picture
• Folder(Long white)
• Fire certificate – Bureau of Fire Department
• Zoning Compliance
• Tourism Registration – Memo Circular No. 147 series of 1993 of the DILG to include the business establishments SSS ID no. of employee
• Food Handlers Permit – for establishment serving food

III. Bureau of Internal revenue
• T.R.U (Taxpayer’s Record Update) forms available at BIR office
• Mayor’s permit
• DTI Business Name Registration Certificate
• Tax Identification Number (TIN)
• Applicant’s Birth Certificate. If with dependents, submit also their birth certificates
• Marriage contract for married applicants

IV. Cooperative Development Authority
• Simplified Economic Survey
• Articles of Cooperation
• Treasurer’s Affidavit
• Cooperative By-Laws
• Bond of Accountable Officers

V. Securities and Exchange Commission (For Stock Corporations)
• Articles of Incorporation

• Certificate of Bank Deposit
• Authority to Verify bank deposit
• Written Undertaking to Change Name(Under Oath)
• Registration Data Sheet
• Date of Birth of Incorporation and Board of Directors
